As the health care community increasingly recognizes that socioeconomic factors have a significant impact on health outcomes, greater attention is being placed on understanding and addressing social determinants of health in an effort to achieve greater health quality. This session will discuss efforts by payers and health programs to understand the impact on healthcare costs of social determinants of health such as inadequate housing and transportation.

Upon completion of this session, participants will:

    • Describe the relationship between socioeconomic factors and health outcomes
    • Evaluate efforts to understand the impact of social determinants and improve the health of various populations
    • Design programs and payment arrangements to identify and address health outcomes impacted by social determinants
